How do I attached an Index to Salt Lake County 3rd District Court Probate Cases?

David Merrill and I created an alphabetical index to the Salt Lake County 3rd District Court Probate record between the years of 1853-1893, when we attended BYU. There were 25 boxes (reels) of films. Our index has about 20 pages.

I own the rights to publish this index and would release it to the Creative Commons license: Attribution-NonCommercial-NoDerivs 3.0 Unported

I would like it online, so it can me put to better uses. It could be added as a waypoint no the films or linked the Wifi page etc. How or who do I need to contact to accomplish this goal?
