India—Madras Diocese Protestant Church Records - surname listed on the side of the record

I know we are only supposed to index what we see and to index it as written. But the typical birth records I'm coming across so far are as follows:


Child's first name only, son/daughter or Father's first name and surname. [surname listed again on the side]


And, every once in a while...

Child's first name only, Father's first name ONLY. [surname listed on the side]


How should I handle this? I feel like the surname listed on the side is supposed to refer to the child because you would think it would apply to the principal person on the record, in both situations. Thus I would think you would index the child's surname in both situations, and you would index the father's surname only in the first situation. But I don't know if that's the correct logic.

I wish some clarification would be on the project guidelines.
