Inaccurate Indexers
You need to establish a specific forum for reviewers to report exceptionally bad indexing. If we report them in normal "questions", there's no guarantee the right people will see it.
So here are two really bad batches that someone in authority should look at:
1 - Philippines, Camarines Sur, Caceres—Registros Parroquiales, 1716–1977 [Parte B] [MMZZ-K2V]
The only thing the indexer did right was label the records as marriages. Otherwise, they put 6 records in instead of 4. They put random numbers in for page and records. Everything else was blanked. Pitiful attempt to look like they were doing something.
2 - Philippines, Camarines Sur, Caceres—Registros Parroquiales, 1716–1977 [Parte B] [MMZF-TWV]
The only thing this indexer did right was put 5 records in. However, they designated them all as baptisms (They are deaths) and blanked everything else. Maliciously lazy?

Thank you for your comments. We are aware of many new indexers and that some batches will not be done correctly. When this happens and they go to review, the reviewer has two choices--either index the batch correctly or return it for reindexing. We appreciate the love and support shown in the indexing program for new indexers.

There is an avenue for reviewers to help indexers/reviewers get some extra help. You can return the batch for reindexing using the form "Reasons to Reindex Batch". You will have to activate the form. With the batch open, click on Help, click on Labs, Activate the form and follow the instructions. Check it out - you can look at the example and see if you would like to use the form.
Returning a batch should for reindexing should be a rare event as explained in the link to the knowledge article. But, if you really believe that an indexer or a reviewer needs a little help, it is a good form to use.

Returning a batch for reindexing does not help the original indexer see what they did wrong. Another (different) person will get that batch for reindexing. I prefer to redo it myself. That way, I know it will be done right the first time. A second reviewer will have to look at it no matter what.
I understand that the old feedback system had its flaws but at least there was some feedback. Nowadays, nobody has the slightest idea what they did right or wrong.

@Jerry Hill you are right that just returning the batch for indexing does nothing to fix the situation
but if you enable the "The Reindex Batch dialog" in Labs as described by Melissa you will be able to give specific feedback that is linked to the indexer (whether they actually get it or how it is used I am unsure)
Just be aware that you will have to refresh or reload the batch to get the feature activated on the batch.
I find that it falls off periodically so I have to redo the process an re-enable it every so often.
There is an option to flag if the indexer is abusive or malcious and another if the indexer needs more education and you get to indicate which areas are in need. It is worth the effort.
