If there is a stamp that says, "Date Filed" with a date included on a document to be indexed, should I put that in as the record date or should I use another date that is on the document?

    You will use the the issue date of 4/1/36

    It's 4/1/36. There is no example for a COA, but going by previous projects, that COA had a "Date Issued" date, which is what the record date should be.
