What do I do with extra information written on a document?

I am currently working on indexing declarations of intention as part of the US, Pennsylvania, Eastern District—Naturalization Records, 1896–1931 project. For context, the indexing project asks us to check to see if the document mentions the principal person's spouse, and to list that information if so. Some intention documents have a section for information about the spouse, others don't.

The particular document I'm indexing does NOT have a section for the spouse's information, but somebody wrote some spouse information in-between the lines of the printed text. Based on the handwriting and the tense of the sentence, I'm pretty sure it was written by the original document person.

Should I index the spouse information, even though it's written very informally, not part of the actually filled out legal document?
