    I do not know the Russian/Hebrew languages - you will probably find a response soon. But for your reference - you can try to pick out Russian genealogical words using this page from the FamilySearch wiki:

    Here is the Google Lens translation of the form headings (which detected Russian language - but could not detect the handwriting):

    Not sure but the part about Number and month appears to be Christian and Jewish calendar dates.

    My husband says he can't read the handwriting well enough, but the headings indicate it is some sort of marriage document.
