Need Permission Form doesn't allow me to complete it

I have my deceased niece's son's permission to have her sealed to my sister, her deceased mother and her husband.

The form doesn't allow a box for her estranged husband to be "unable to locate". The form won't allow me to submit it with the information about her son who gave me permission. WHAT do I do??
