The right column of burials lists - Samuel Francis of Pendleton. The way the "of Pendleton" is used appears as if it is part of the surname. His son, Samuel, shows his parents are Samuel and Mary "of Pendleton" The Francis isn't even used. During this period of time, many although not all, families didn't have surnames. So what do I enter where? I thought maybe putting Samuel Francis as the given name and blank for his surname. Since I'm not sure if Francis is his given name or his surname. I also thought to use Pendleton as his surname or "of Pendleton".

I would index Francis as the surname. There are several deceased: Samuel Francis, 62 years old. Mary, the beloved wife of Samuel Francis (72 years old), and Samuel, son of Samuel and Mary Francis who died at age 9 years and 7 months. There is also a grandchild, Samuel Francis of Pendleton, late of Wrexham, who died at 74 years old.
I think it is clear when they use Samuel and Mary Francis as the parents that Francis is their surname. Also in the first record it says James Kilner of Monton, so they are most likely using "of Pendleton" and "Of Monton" as place names.
I wouldn't index Lancashire as the burial place since it is not on the document and we don't use information from the title, except sometimes to determine the first two digits of a year.
