What do we do with "Additional Information for Death Certificate"?

While indexing death certificates, the fifth image in the batch is a form titled "Additional Information for Death Certificate." It includes the name from the previous image, city, and the death date, but none of the other information asked for in the indexing form. I'm quite sure its the same person from the previous image, (which was the "standard certificate of death" for Massachusetts. It included additional information about surgery before death, but doesn't include any indexable information except registration number, name, city and death date. (All of which were on the prior page) Do I index this as well, or not?

    It is important to share your batch when asking a question to get the best answer. To share a batch, click on the third icon from the right and follow the instructions.

    I happen to have a batch of these Massachusetts death certificates in my queue. The instructions say:

    • Index all records of death, including standard certificates of death, medical examiner's certificates of death, and additional information Forms.

    You should go ahead an index that form in addition to the standard certificate of death. The example of the additional information form also shows that it only includes the reg number, name, city, death date, and gender.

    Thank you so much! I obviously stopped reading too soon when I was looking over the project instructions. Thanks for calling my attention to it! And for the advice on how to share a batch. I'm new to all this.
