Hello Karen Ann and Welcome to Indexing Chat! You are correct and would only enter John Smith once.

For the purposes of the index that we are creating, it does not matter how many John Smiths are listed on the page, a single entry in the index will allow a researcher to find all of them. Apparently for the purposes of the record custodian a single instance of a name that appears multiple times is also sufficient.
