How should the maiden name be indexed?

US, Wisconsin—Marriage Records, 1940–1972 [Part B][M3MB-ZKC] In this project for the maiden name of the bride, under the name of the bride on the next line it states "Maiden name if Different". Should we go ahead and put in the maiden name field the last name of the bride if that line was not filled out?

    Only record a maiden name if it written as Maiden Name on the form. If there is a name in the box that says, "Maiden name if Different" you may record that. The Maiden Name box is just below the bride's name or to the right of the bride's name or below the bride's mother's information.
