How do I index a surname and maiden for the spouse?

US, Wisconsin—Marriage Records, 1973–1978 [Part A][MS1F-BFS] I am reviewing the first image and the indexer put the surname of the groom in the spouses surname field. Shouldn't this be the surname of the bride listed? And they also put the bride's maiden name in the field for maiden name, which on the document it was left blank. So should that be marked "Blank" in the maiden name field?

I have access restricted, so this is from memory. Many indexers have put the groom's surname for the bride's. That is incorrect. The bride's surname is her last name on the form. Only list a maiden name if it is specified on the form as Maiden Name. Also, as a side note, the groom's maiden name is USUALLY marked <Bkanj> but on some of the forms there is a maiden name listed for the groom. I had to reread the Field Helps to figure those out.
