Ordinance request bug, may be related to 'permission required' or maybe because person has multiple

I've explored the change log and the memories. What is interesting is that her own account, written while she was in her 40s, is buried in a single story. That needs to be split out and then used as a source in the source list.
If the her handwritten (or typed, if she typed it) account is available, that should be scanned and uploaded as a memory. It explains while there are three sets of parents, though the Burbank family is likely not related to her, either by adoption or some other means. They may have run the home in which she was placed, but normally, she would not be sealed to the operators of the children's home. Nor, would they be listed as her parents (which they are).
If she provided an audio recording (or video), the I would extract the audio portion and upload that to her memories, along with an accurate transcription of the audio.
So, what is missing in the sources -- a record of her in the children's home. That needs to be chased down and included. My wife's mother was placed in a home in Boise and we happened to stop at the home and they dug up the record and gave us copies.
Official adoption records. The home may have these, as well. They should be available since she and her sisters were older at the time they were placed in the home. If not, you may have to take her account and see if you can get the adoption authority in Denver to find the adoption records... there should be a court case, as well.
Birth certificate. This should be available, but again, you may have to chase this down in person, carrying her account of her life with you.
You may also want to check the Denver Public Library -- they have excellent genealogical records and may have local court records in their collection.
To get back to your question about permission.
The first four ordinances were performed by the person -- she was a member of the Church and recorded the fact in her account.
Next, in looking through the change log, it turns out the she was adopted. Therefore, sealing her to her biological and adopted parents is correct.
You were moving ahead on sealing her to her biological parents. I would go ahead with that.
She was subsequently adopted and her adopting family joined the Church when she was seven. This is all detailed in her own account including her own baptism.
Because she was already a member of the Church, there is no need for permission to get her sealed to her birth parents.0 -

I appreciate the suggestion on looking for adoption records! I was able to find some info about making a request with the state of Colorado which I will follow up on this week. My sister had tried in the past without success but it looks like records may be more available now.
But back to the main question which I hope familysearch team can fix is that when I try to request the sealing to parents ordinance, a message box pops up saying 'Completed' and prevents the request from being 'granted'.0 -

If there are multiple sealings to spouse or sealings to parents and one of them is incomplete, the only way to see it is to go to the 'ordinances' tab.
I think it has been like this for many years, but is this the ideal way to show ordinance status? Maybe if there is still an incomplete ordinance the icon should show as green or yellow.
