It is actually a transcription of a record from Alberobello, where she died. The original is here, #224:
Died 16 Nov 1886, age 2 at #51 Vicolo Perrini. Born in Monopoli. Parents Mariantonio and Margherita Nardone.

In the year 1886 I, the undersigned Giovanni Colucci, reader delegated by the mayor with a deed dated January 31, 1883, having today received an authentic copy of the death certificate from the registrar of Alberobello, I have fully transcribed the aforementioned the copy which is of the following tenor.

the year 1886 on November 16 at 10 am and 10 pm in the Municipal House: In front of me Vito Crevisani, secretary, delegated by the Mayor with deed dated 23 December 1881, duly approved, registrar of the Municipality of Alberobello - are appeared Martellotta Marcantonio aged 48, farmer, domiciled in Alberobello, and Pagliarulo Nicola aged 27, bricklayer, domiciled in Alberobello, who told me that at 12 am and 50 minutes today in the house located in Vicolo Perrini at number 51, Martellotta Lucrezia, a 2-year-old child living in Alberobello, born in Monopoli by Marcantonio, a farmer domiciled in Alberobello and Nardone Margherita, a farmer domiciled in Alberobello, died.
At this act are present as witnesses Labate Domenico, a carpenter, aged 45, and Covertino Domenico, aged 41, a carpenter resident in this municipality -------
Having read this deed to all those present, the only witness Covertino signed with me to be the other illiterates
