I still haven't been able to find consensus for putting in periods after initials or commas when needed. Anybody know the answer to this?

    Yes, you will include punctuation like periods after initials. I'm not sure when you would encounter a comma as part of a name, but if it is part of the name include it. If you see Smith, John you won't add the comma.

    Do we only put the periods after initials if they are on the document itself, or do we use them after all initials? I've been trying to put exactly what I see on the document.

    Put what you see on the document. If the document has a period, include it. If it doesn't, don't add one.
