Prefix information
When the term physician follows a persons name, do I assume and put Dr in the prefix or leave the prefix blank.

I'm not sure that always applies in real life…

Unless it has Dr. in the listing, leave the prefix blank. Physician is his occupation and we do not assume anything in indexing.
