Sure names on the city directories. If more than one surname shown are they to be entered

    When you see something like Smith, see Smithe or Smythe, you do not index that. That is a reference for the reader to see that if you cannot find it under Smith, look under the other names. What you will index is the Smith surname only.

    If the original poster means something such as (making up a name here) Mary Francis Smith Collins, then Smith Collins would both go into the surname, would it not?

    Yes, it will. The same maiden/married "rule" of putting maiden first applies to all projects if the surname is Smith Collins.
