Making sure I'm doing this correctly.

On image three, leave the date blank (don't take it from the AOW on next page)?

Also, as the instructions say "For this project, please index additional document types including certificates of arrival, certificates of examination, affidavits of witnesses, and depositions," I will enter two records for image four, AOW and OOA.

Image 3 continues on Image 4. The record date follows Subscribed and sworn to before me. The record date for Image 3 is 12/13/1929.

We haven't heard about the ambiguity of the AOW statement that is attached to the PET. I would not index it based on the fact that there are stand-alone AOW's in this project.
