With no heading, can anyone tell if this is a death records or baptismal record?

The word is almost the same "baptizat_" - the lower 2 are baptisms. The top one is a conversion - "received from Anglican church."

Thank you. I have received 2 batches to review and they had both been marked as death/buriel records. I couldn't figure out how they got to that and doubted what I was seeing. Appreciate your help
