    The second image is the Oath Of Allegiance, which is below the Affidavit of Witness section. The Oath is its own, separate document that is indexed with only the applicant name and date. Post-1940 Petitions have both on the back of the document; pre-1940 Petitions have the Witness section on the front and the Oath by itself on the back. You can see an example of this under Project Instructions/What To Index/How To Index an Oath Of Allegiance.
