reviewing an indexing record

Im reviewing an indexing record and many of the records are not there , the unreadable records are not listed, it just goes to the readable records . Do I need to document all the unreadables

I wouldn't index the unreadable records. However, because this batch has several errors in the first 150 entries, which have not been fixed (incorrect usage of punctuation and surname errors), I'd throw it back for Reindexing.
