US, Rhode Island Nat Records, part G. Image 2 appears to be a continuation of Image 1.

For Image 2:

1) Do I only index the name Napoleon Chagnou? or mark it NNED?

2) Did I spell the last name correctly? or should it be Chagnon?

3) What date should I put for record? Or mark it blank?

    I would mark it as NNED. The last name is Chagnon. You can see the second "n" on Image 2. According to the PI, the record date is at the top of the document, so 9/26/1906 will be your record date.
