How should this name be indexed ? Should the former name be included with the birth name ?

    The instructions for that field found by clicking the purple question mark state:

    Maiden names should be typed in this field before the current or former surnames. A maiden name was often indicated by the words "geb," "gebore," "née," or "born as." A maiden name may also have been written in parentheses. 

    If I were to follow those instructions I would type, Jensen Merkle Witcher, in that field.

    Hope I am correct.🙂
