Ability to index has been denied
What are the reasons why FS denied a contributor doing indexing/review? He has done it carefully and followed instructions until he reaches a million mark review. How could he get back his indexing/review rights without worrying his familysearch account will be locked out and temple work will be denied too.

I would say it is unlikely the person has had his rights revoked. It could be a glitch with his system. Has he tried clearing out his cache, browser history, and familySearch cookies?

Good news on his being able to index!
If his totals show that he has indexed 1000 records in web indexing, then he should be able to review. He can't fix a missing review tab. That would involve a call or email to FamilySearch to figure out the reason why the review tab is missing. Records indexed or reviewed in the old desktop program don't count towards the 1000 records.

He is already a reviewer, but suddenly, review tab disappeared. With a million in his indexing totals.

That is definitely going to require a phone call. I lost around 180k that were never recovered, but, a million - that is definitely a problem. Click on the Question Mark in the right corner and Contact Us for all the ways to get in touch with FamilySearch.

He contacted familySearch and was told that the engineer does not know how to fix it yet. We will be waiting for the update.
