On image 2 did I index the name correctly on the Oath of Allegiance?

I put:

given name: Jacob or Fred

surname: Koppelman or Jacques

****Please check image 1, After doing image 2, I am thinking that Jacks or Jacques should be in the surname field?

    For the OOA, you will go by the signature(s), so you did this correctly. However, on Image 1, the Surname will be Koppelman Or Jacks or Jacques Or Koppleman.
