Image 1 is fine. For Image 2, the Record Place is Brooklyn, New York and the record date is located in the affidavit of witnesses section on the +1 image in the Reference Images.

@erutherford thanks for your review/input. I didn't even think to check the reference images. I don't remember reading anything about using them in the PI. Again, Thanks!
