Lookup Request Form print problem

I recently used the FamilySearch Library Record Lookup Service Form. At the end of the form you can print out your request. Problem is that it prints on 3 sheets of papers with huge gap of about 2 inches between each question. Can you fix this so all infor/request prints out on one sheet of paper instead of three?
