Did I complete the name fields correctly for the "spouse" on image 5?

This is the Wisconsin Divorce records. On image 5, the wife's last name is the surname of her husband but it is written in the Maiden Name box, so I marked her surname as BLANK. Is that correct?

I am not authorized to view that batch. From your description I would recommend entering the name as displayed in the record - not using <BLANK>.
