US Tract Books – which names?

In Sample 2 for Tract Book Records, both the name on the top line, Charles Slade, and the next name on the page, James Hill, are indexed in one transaction. What about the following line, John Clark on 1 Feb 1837 – does this transaction (and its index) also include the name at the top, Charles Slade, or just the one name?

    Please show the batch number at the end of the batch so someone can help you. Such as[xxxx-xxx]

    And I wonder if we should use that name from top of page again with each entry on the page. And is Dr. a prefix or is it an abbreviation for something else?

    The example I gave was from the Project Instructions. The batch I am working on is [M3CV-2VN]. In this batch, the question would relate to whether the name at the top, Edward Turner, should be included in the entries for all other names in the image.

    From what I've read, Dr. is not part of the info to be indexed.
