How can I get an original source aligned with the indexed info.?
The indexed version of the death certificate for Frances E Cummings, fits another death certificate, not Frances's. All the indexes seem to be off. Here is the source:
"Massachusetts, Town Clerk, Vital and Town Records, 1626-2001," database with images, FamilySearch (https://www.familysearch.org/ark:/61903/1:1:FH47-SS4 : 10 November 2020), Ellen Cummings Barry, 17 Feb 1903; citing Death, Boston, Suffolk, Massachusetts, United States, Massachusetts Secretary of the Commonwealth, Boston; FHL microfilm 007009424.

I want to save this source in Frances's and her mother's, Mary Newman (Noonan?) sources. but the way they are shown presently will just confuse future researchers.

Yes, the index is off from the image. On the Image viewer for the Source - there is a Source Box button (above upper-right of the view area). You can save the Source to your Source Box and create the Source there and then link it to the family.

The index-to-image associations are off by one. Frances's index entry is associated with the next image.

It looks like the index has already been attached to the profile, so the easiest thing to do is to edit those attached citations to add a note about the correct image number (642). You can, of course, also attach the image directly as a source, as genthusiast explained, but that's optional.
