Reviewing. Lots of entries all filled out that I don't see on the document? Only one image?

Without your batch code--which is in the second set of brackets that goes with the project name (I would provide M38M-KYY if I had a question regarding that batch)--the indexer may have entered information from a reference image.
To access the reference images, click the 4th icon on your vertical toolbar. This will open a dialog box that will show your batch plus five images behind and five images ahead of your batch. You can zoom in/out in order to find Mr. Meyers. If you locate him, and verify that the rest of the extra entries are the same, delete the remaining entries.

If the project has a preset number of records, some indexers don't know that they have to delete the unused entries to get the batch to submit. Then they use the reference images to fill in the excess entries. Then there are others who will go to the reference image just to capture whatever information is needed for the last entry and just keep going! Since your screen shot says Image 1 of 1 and we know there are 16 entries, you should delete the remaining entries.
