Under project instructions on "How to index a death record" sample, the sample record they're using shows a place of birth, which we are to enter in indexing these records, however, the sample has marked it "blank", I believe it's the first sample shown, may someone fix this please, thank you!

If there is information missing, then you mark that field blank. Not every batch--heck not even entry in every batch--will have everything that is a required field.

I am not seeing an example like you are describing, BoyleEllenLouise1. Do you have a shared batch code so we can look at it? The Shared batch code is the 7 digit number and letter code at the end of the Project Name in brackets.

When indexing death registers for Washington State, should obvious duplications be included?

I'm talking about the samples, not any particular batch, on the sample image it shows place of birth, but on "how to index this image" they marked place of birth "blank" this particular sample is wrong, never mind the project is now gone, it was there when I typed my comment.

It was the completed Part A. The one record they indexed on Part A had no Birthplace. There were others that did have a birthplace, but, they didn't index any of them as an example. Part C has a birthplace indexed on both examples.
