Questions Concerning Header Review

I haven't indexed in a while and I'm trying to refresh myself to make sure I review everything properly. I'm currently doing Header Reviews. I would usually refer to a page with a list of project rules but other than the field help page, I can't find much else. These are my questions:
If there is punctuation included in what needs to be index such as Ruth F. Somner, do we include the punctuation? I'm also asking the same question regarding the E.D. Number field. Do we include the hyphen.
For Date Sheet Started, do we include the comma and year or just what's written in the blank space.

@kalh7338, we understand that you would like to know about the punctuation after the initial F. Include punctuation that is used in the person’s name. If you are making a correction, try to type exactly what you see on the page.
Regarding the E.D. Number field, please include the dash as the instructions say: "Verify the district as recorded in box d". We make sure that the box matches what the document says.
For the Date Sheet Started, record or verify what you see on the document in the field. "Verify the sheet started date in the order it was written in box f." Since it is a little unclear to include the ",1950" since it isn't always highlighted, I always include it. In that last section under "h", verify or put the date as it is recorded on the document. You don't need to have the year included because it is typed out below that date already.

A portion of my question was answered in another discussion. We are to include punctuation that's written in the header review.
@AndLinda Could you please answer my other question about whether to include the comma & year when recording the date or only what's written in the blank space?

@AndLinda On a census page I was reviewing, the E. D. Number was 623. The computer instead entered 6-23. I pulled up the census page and the computer is correct, the E.D. is 6-23. Do I change the E.D. to reflect what's on the census page or leave what the computer has entered in?
Another situation happened where the Incorporated Location box had Clermont Harbor Beat ?? listed while the computer had Bay St Louis entered for the location. I looked up the locations and Clermont Harbor is an unincorporated town that seems to be located within Bay St Louis. So I guess the computer info. is accurate but what should I do? Should I change the computer's info or leave it as-is?

@kalh7338, if the census page or document has the hyphen then the box should show the hyphen. You verify or correct what is in the box to match what it shows on the census document. As far as the incorporated location box on the document, verify or change what is in the computer index box to match what is on the document. I'm sorry it is so confusing. Do the best you can.
