I am doing birth records for Wisconsin- birth records. I am not sure if what I'm doing is a birth "certificate" or birth "charge-out" sheet. I have done a few sheets wrong. Hopefully, it will be all figured out.

The Birth County is Milwaukee --- Only Image 3 should be marked Amended . Please fix all the entries to Images 1, 2, 3 or send back the batch for re-indexing. Please do not submit this batch.
