What is a charge-out in indexing birth records?

I don't know the difference between certificate and charge-out.

    Check the project instructions and examples for how to index/review charge-out forms. In general, some charge-out sheets are a record of an adoption or other information added by the state sometimes months after the child is born. These do not always show the birth date or place, parents' names or other information shown on a birth certificate. Charge-out forms are labeled "Charge-Out" at the top of the form.

    Mark the Suffix <blank> when there is no Suffix like Jr. Sr. etc

    The project instructions don't show an example of a charge out form.

    @carrieelisebaker1 -- Are you working on Wisconsin Pre-1907 birth certificates? This project doesn't have any Charge-Out forms to be indexed, and I believe they did not use charge-out forms before 1907, therefore no examples are needed. 😎
