I need help understanding what to do with the name the arrow points to. Do I index this name or not? It is coming from a Tax Book. I haven't found anything in the instructions to help me and I'm not sure what the "ux" means.

et ux means and wife. et vir means and husband. et al means others.

I think they are trying to say Belle is the wife of Alfred T.

I would create a record for Alfred T Wetmore since you are indexing all the unique names.
