As a consultant am I able to fill out the FHC Monthly report? Where can I find it and how do I submit it?

I serve in stake Presidency of the Cagayan de Oro East Stake. I was wondering how to get the monthly report of names submitted to the temple. We are setting goals and we can't measure up because we have no idea how many names are submitted for the month.
