How do I find this record? More importantly, From what I understand it is just and index, I would like to find the actual document the index is associated with. Is it a birth certificate?

It is a birth register - each child is listed on a single line. Many locations used the register format in early years of registration.

https://www.familysearch.org/ark:/61903/1:1:VNWG-3T1is the register page, and Max is #156.
