Parents petitioned for naturalization of an adopted child with a name change

I have a record where parents petitioned for naturalization for an adopted child. They provide the new name they want their child to have in addition to the name the child had when she arrived in the U.S. Do I include both given names and surnames in the Given Name and Surname fields?

    Yes would show both names when your index them (Ex: Patricia or Mary Teresa). Your would index the last names the same way.
