This baptism record seems to have two wives listed? Just wondering which to index - the index only allows me one mother.

15 [November 1829] John of John Tohey & Gudy Ryan & [?M]ary Ryan

Cully [location/residence] Spons John Quigly & Mary Ryan

James Ryan

Am I to assume that Mary Ryan the sponsor/godmother was accidentally listed twice?

In this case the pattern is fairly well known - James Ryan is the priest listed a little below the baptism.

What is unusual is the &Mary Ryan on the same line as the parents (it makes her look like a second wife)

but I assume for whatever reason - she was listed twice - maybe this is a copy of the original record and the scribe just wrote her down twice ... i've gotten lines or something out of place when transcribing an original record before. (after a while transcribing your eyes go crossed)

With each Catholic marriage there is a sponsor or two. Usually there is sp or sps before they are listed. James Ryan is probably a sponsor for this wedding.
