@LarsonSusanEllen1 Thank you so much for helping FamilySearch to index records. In order to help you better can you please share the project name and batch number? Below you will find directions for finding the batch code.
Here are the steps to follow to share the batch code:

- Just above the toolbar, click Help.
- On the drop-down menu, click Share Batch.
- The Share Batch window opens, giving you two options:
- Copy the batch link, and paste it in an email to the helper or post it on the Share Batch, Etc. page on Facebook. The helper can click the link and go directly to the batch.
- Share the batch code so the helper can enter it in the web indexing tool and open the batch.
- A second way to find the batch code is by clicking Batch, then About Batch. The Batch ID is the same as the code.
Here is an example: US, New York, Suffolk—Index to Land Records, Grantees, 1630-1975 [Part A][M3WB-588]

The birth name is the maiden name and then you have the married name. Index as: Maiden name Married name. Shown both in Surname box.

Related question here: If the Project Instructions say (something along the lines of) - don't suppose surname from any other persons listed... doesn't this mean we only input the name listed?
Why would a Project - that says not to suppose surname also say - input 'maiden name married name' - isn't that 'supposing' - I haven't indexed a record yet that has maiden name married name on the mother in the record?
Isn't the general rule - to just put what is on the document more correct?
It's also confusing if this sort of thing happens mid-Project - that means fine tooth combing through the instructions each batch rather than supposing the Project hasn't changed...
I mention this because this appears to be what happened in Cork, Ireland church records Project. I open a batch that already had work from a previous indexer and the mother's surname field has two names - maiden married -. I look at the Project Instructions and - glory be - they've changed.... I didn't realize that perhaps the batches from the same Project have different instructions...

@genthusiast This is why the batch code is needed to get the correct answer. There are some Project instructions that tell us not to suppose the surname from another person listed. And, there are projects that ask for both the maiden name and the married name of a woman. The project instructions would supersede the General Indexing guidelines.
Projects instructions vary in projects and batches depending on what the record holder has requested. They can change if errors have been found. So, they should be reread occasionally. Keep in mind that the most accurate information will come from current project instructions in the batch. Instructions can change from batch to batch.
