Reviewing Indexed records?

I have been reviewing record but the last couple of records that I have pulled up don't match the record that is attached. The indexer seems to be indexing some other page than, what I am seeing. How do I get this information to the technical team to get this corrected? I have had to completely re-index the documents in this project.

We are reporting this for malicious indexing since the names seem to be just made up. They are not in the reference images either.

I don't think I got credit for a couple of batches I did. How can I check back and see that I did or didn't?
