Look up service form down

I attempted to use the request a look up and the request form says it is closed. Hope this can be fixed as it is a valuable service. @Áine Ni Donnghaile That does answer my question but that old bad link should also be fixed in that article if you can pass that on to the correct department, because I am not the only one having that issue.

That's already been done. Maybe you need to refresh your link to the article. The correct link is there in 2 different places.
