Where is the indexing form

I am in web indexing, I have read the project instructions and answered the question "Should this image be indexed? However, I cannot see and do not know where to find the form I need to fill out.

So, where is the form and how do I access it.

    Once you click through all of the images and answer the question, Should this image be indexed, the form should pop up.

    There are a couple of really great videos for new indexers:

    This is a seminar with Wendy Smedley and Jason Pierson. The indexing tutorial starts about the 8 minute mark, although it is great to watch Jason talk about his family and his job at FamilySearch.

    @Squander Sue Newell If the form is not there - perhaps the Table is. I have noticed a couple Projects are sending out as Table default but if you like form - you can change it in the Data Entry menu:
