How did I get the back side of death certificate that refers to notes over on back of certificate.

    Hi Cynthia.

    I would think that that the note that says to "Read Explanations on back carefully" have to do with the completing all fields that you see on the front. In other words, they are instructions for the person who is filling out the death certificate, not necessarily for the recipient - you! All the information you need is showing clearly on the front.
