What does the 4 mean beside the babies name in this Swedish birth record?
I am looking at #604 George Gosta. Does the 4 mean he is their 4th child?

I think it is probable that the number following the child's name is the number of children born to the mother. I have found 2 other children born to this couple using Roteman's Archive 1878-1926. There could be a third. Roteman's Archive works very well for searching in Stockholm during the time period you are looking at. If you search for "Olga Ros%" in the "Förnamn" field and with the birth year "1865" in the "Födelsedatum" field you will find the page I am attaching.
The other 2 children are Olga Elisabet, born 6 March 1888 (https://sok.riksarkivet.se/bildvisning/00025629_00030#?c=&m=&s=&cv=29&xywh=-529%2C20%2C7300%2C3919) and Nanny Margareta born 17 December 1889 (https://sok.riksarkivet.se/bildvisning/00012310_00103#?c=&m=&s=&cv=102&xywh=-125%2C2962%2C3496%2C1877). I will attach an image of that document. It also shows she is a "Skom. hustru" (shoemaker's wife) and that she had moved to America in 1890. Apparently she returned and had a child in 1894 in Katarina parish, Stockholm. That is confirmed in Georges Gösta's birth record-after her name it reads "tyn N. Amerika", which I believe means she returned from/left North America.

I am a little confused. According to this, Ola Jönsson Evander and Olga Rosalie Pettersson are living in "Brooklyn (NY), N. Amerika", and yet, there is an address in Stockholm for them-Södermanengatan 19. Either they returned to Sweden for the birth of the baby (which I doubt), or they reported the birth to the parish. Since there are no witnesses mentioned for the baptism, I would think that the second option is probably correct.
Also, there is a notation "Feke döpt" in Column 23. I don't know what that means. I am going to ask @Morris Geoffrey Fröberg to look at this and see if he can figure out what is going on.

This is an interesting puzzle. I looked up the family in the Roteman 3 database (the cd version of the Roteman's Archive 1878-1926 database on the Stockholm city archive). In looking at the entries for Ola Jönsson Evander and Olga Rosalie Pettersson (Evander) the roteman records show that Ola emigrated before Olga. The roteman records state that Ola Jönsson Evander moved to Hedvig Eleaonora in 1887 to the street address Biblioteksgatan 13. Then to Valhallavägen 37 in 1888 still in Hedvig Eleonora and then to the U.S. in 1889. Olga moved to Hedvig Eleanora in 1889 and then to the U.S. in 1890. This agrees with the note to the far right in the birth and christening record that Norm found.
As far as the birth record, the birth of Georges Gösta is recorded in the Katarina parish birth and christening record. It states that the residence of the parents is in Brooklyn N. Amerika. It also includes the note that Georges Gösta is the 4th child to the mother, and "Icke döpt" means not christened. From the roteman records we know the parents emigrated to the U.S. in 1889 and 1890. So there are two possibilities, either they went back to Sweden for a trip and she gave birth there, or they notified the parish of this birth.
To the question of notifying the parish, the next question is why? There was no legal obligation, advantage, or benefit to notify Katarina parish. Further, the level of detailed information in this record supports the idea that they were in Stockholm at the time of the birth. The roteman offices in Stockholm city were the legal vital records offices for residents living in each district. I looked to see if Georges Gösta's birth is recorded with the roteman records. He is not. This supports that the couple (or perhaps just Olga) was in Sweden when the birth happened and it was not reported because they were not citizens of Stockholm city. They were Americans visiting Sweden. It's interesting that the birth year of Ola Jönsson Evander is listed as unknown with a ? mark because if he was there, the information could have been gathered easily. Yet they recorded the birth date, marriage date, the residence in Brooklyn, and reference of 4th child would easily answered by Olga. I think the midwife who assisted the birth gathered the information from Olga and reported it to the pastors office for Katarina parish. Once it was recorded in the ministerial book, the information was extracted to the civil extracts which you originally found. As far as Olga traveling to Sweden. We don't know when she arrived. Seemingly she was staying with someone at Södermanagatan 19. Travel to Sweden at this time was affordable and relatively fast since they were traveling by steam ship.
