What is the right thing to do?
I have a family that has a foster son. I can see that I can make the parental relationship be "foster" parent, but my question is should I make a record of him with his mom? And would the last name be of his birth mom or the foster family?

#3 Anthon Elof's birth record. I can see his mom's name is Carolina. I think the last name is South , though it doesn't sound very Swedish to me.

I believe genealogy is all about information. I put all "parents" applicable on a child as that gives a window into his or her life. When I don't know the relationship because the child is just with the family in a census record, I will establish the relationship "guardian". Going back to the era of your records, it is possible he is more of an apprentice than a family member. I have a female ancestor in the same situation, slightly later than your situation, 1850. I have thought about this situation, and I may give my female ancestor 2 relationships: guardian and an Other relationship using the apprenticeship connection. Do you think that could apply to your situation?
Just to give you an example of KNONW relationships: I have an adopted relative who has 3 sets of parents: birth mom and her spouse, birth dad and his spouse and adoptive mom and dad. I like this because siblings can easily be seen as paternal half siblings, maternal half siblings and adoptive siblings. One of my husband's ancestors lived in at least 3 households while he was growing up. Aside from his parents, he also lived with the family of an adult step sibling (father's second wife, head of house was child from her first husband), and on another occasion I found him with distant blood relatives. I have them listed as "biological" or "guardian".

The last name given for Carolina starts with "B" not "S", cf. Bengtssons in the record above. But even "Bouth" is a very unusual Swedish name if it is correct. You should try to find another record for her to confirm the name.
Note that Anthon Elof was born illegitimate in Laholm and his mother Carolina was apparently from a different place - the birth record says she is from Skåne which is the neighbouring province with no specific parish mentioned. So it might be that she came to Laholm only for the childbirth and Anthon Elof was directly afterwards placed into foster care.
