Gevatteren waren: Johann Adam Conrad von Riesweiller im "Chur Pfältzisch" [Kurpfälzisch] Ober-Ambt Alten Simmern u[nd] seine ehefr[au] Helena.

Translation:

Johann Adam, child of Johann Adam Hofmann, joiner here, and his wife Anna Catharina, was born on the 23rd of August and was baptized on the 29th of the same month.
Baptismal sponsors were: Johann Adam Conrad from Riesweiler in the administrative district of old Simmern of the Electoral Palatinate and his wife Helena.
