I don't think this is a death record. What I read is:

which I think translates to:
The 19 Nov baptized Adalbertum born 6 Nov parents Adolphi Noll and Alidloniae (sp?) Thoma [two words here I couldn't transcribe] Patrons Franciscus Schreck and Libra Noll. Res. Crooked Creek, Jefferson County.

IMHO you should take this to be translated by someone who speaks German. "Die" is a German word that means "the" in American English....so the 19th of November...It might be the baptism date, the birth date, the name of the parents, and then the county of their residence.
