I think the letters are actually P.P. meaning "patrini" in latin or "padrini" in italian. This refers to both the "padrino" = godfather and "madrina" = godmother. The custom was to have one or two "padrini".
If they were related (sister, brother or wife), this was often recorded, as well as if they were attending the baptism "representing" somone else - usually some VIP who could not attend in person.
